Dear Former Patient,
Somnatek Sleep Health Center ceased all clinical operations on September 18, 2014. We wish to inform you of the arrangements that have been made regarding your patient records and any outstanding account matters.
Records Transfer
Patient records from Somnatek Sleep Health Center have been transferred to Dorsal Health Holdings LLC in accordance with applicable state and federal requirements. Dorsal Health Holdings LLC will maintain your records for the legally required retention period.

Please include your full name, date of birth, and approximate dates of service when submitting a records request. Allow 15 to 20 business days for processing.
Billing and Insurance
All outstanding billing matters, explanation of benefits questions, and insurance coordination inquiries should be directed to Dorsal Health Holdings LLC at the contact information above. Somnatek Sleep Health Center no longer processes billing inquiries directly.
If you received a billing statement dated after September 18, 2014 bearing a Somnatek return address, please contact Dorsal Health Holdings LLC to confirm the status of your account.
Prescription and Referral Continuity
If you require continuity of CPAP or other durable medical equipment, or if you need a referral to a current sleep medicine provider, please contact your primary care physician. Somnatek Sleep Health Center cannot provide new prescriptions, referrals, or clinical consultations.
Research Study Participants
Former participants in the Wexler University longitudinal sleep recall study should direct any questions regarding their participation records to Dorsal Health Holdings LLC. Research participation documentation is maintained separately from standard clinical records and is subject to applicable research retention guidelines.
